The PANORAMIC Study

The PANORAMIC Study is a clinical research project being conducted at The BMI Clinic in Sydney. It aims to evaluate a new, minimally invasive procedure for adults living with Type 2 Diabetes.

The procedure, called Duodenal Mucosal Regeneration (DMR), targets the lining of the upper small intestine (the duodenum) to help regenerate healthier tissue - with the goal of restoring the gut’s natural ability to help regulate blood sugar.

Type 2 Diabetes & the Gut

Type 2 Diabetes is a progressive condition that makes it difficult for the body to maintain healthy blood sugar levels. Over time, it often becomes harder to control — even with diet, exercise, and medications.

The duodenum plays a key role in how the body responds to food. In people with T2D, the cells that line this part of the gut may send abnormal signals, disrupting blood-sugar control.

The PANORAMIC procedure aims to gently remove these unhealthy surface cells and encourage the growth of new, healthy ones — helping to “reset” the gut’s role in glucose regulation.

Researchers hope this may improve blood-sugar control and reduce reliance on medication.

About the Procedure

The study uses the E2 Endoscopic Ablation System, an investigational medical device.

During the procedure, a thin, flexible camera (endoscope) is passed through the mouth into the small intestine while you are under general anaesthesia.

The study doctor delivers short, carefully controlled pulses of electrical energy to remove a thin layer of the intestinal lining. This triggers the body’s natural healing process, allowing new cells to grow.

The procedure usually takes less than an hour, and most participants go home the same day.

No implant remains in the body, and participants follow a short, guided diet while the intestine heals.

What Participation Involves

If eligible, your participation will last about 13 months, including three main phases:

  • Screening Phase – A health review and preliminary blood tests.
  • Procedure Phase – Baseline testing and the PANORAMIC procedure under general anaesthesia.
  • Follow-Up Phase – Around 9 visits over 12 months to monitor recovery, blood sugar levels, and general wellbeing.
 

Who Can Take Part

You may be eligible if you:

✔️ Live in or near Sydney

✔️ Are 22 - 70 years old

✔️ Have had Type 2 Diabetes for less than 10 years

✔️ Are not using insulin

✔️ Take (non-insulin) diabetes medications

✔️ Have an HbA1c between 7.0% and 10.5% (confirmed at screening)

✔️ Have a Body Mass Index (BMI) between 22 and 45 kg/m²

Costs & Reimbursement

Participation is free, and all study-related procedures, visits, and tests are provided at no cost.

Participants will receive stipends to help cover travel and time for each completed visit.

While individual benefit cannot be guaranteed, participation may provide:

  • Closer diabetes monitoring and education from a specialist team
  • Early access to a novel treatment approach
  • The opportunity to contribute to research that could help others with Type 2 Diabetes

Important Information

Participation is completely voluntary. Choosing not to take part will not affect your regular medical care.

The PANORAMIC Study is conducted under oversight of the Bellberry Human Research Ethics Committee in accordance with the National Statement on Ethical Conduct in Human Research (2023).
