A popular procedure is the Gastric Sleeve (medically called a Laparoscopic Sleeve Gastrectomy) which is a weight loss procedure that works to reduce the size of the stomach by approximately 85% using a stapling device. By permanently removing part of the stomach, the remaining stomach forms a tube (sleeve).

The gastric sleeve is the most-commonly performed weight loss procedure in Australia. The surgery is performed laparoscopically (keyhole surgery), typically requiring patients to stay in hospital for only 1-3 nights after the procedure.

Risks of Gastric Sleeve

While we do provide this procedure, it's important to consider the possible risks and complications associated with having this form of surgery which include:

Endoscopic Sleeve Gastroplasty (ESG)

The Endoscopic Sleeve Gastroplasty (ESG) is a non-surgical weight loss procedure that involves the placement of stitches from inside the stomach to lessen its volume and change its shape, forming a tube.

The ESG procedure was pioneered by the world famous Mayo Clinic in the USA in 2012, and has since grown in popularity due to its minimally invasive nature which reduces patient recovery times, rates of complication and side effects.

The BMI Clinic’s Dr Sartoretto was the first to perform the ESG procedure in Australia, and remains the most experienced proceduralist in this field in the country.

Dr Adrian Sartoretto is a Consultant Gastroenterologist & Hepatologist with a subspecialty interest in Bariatric Endoscopy. He completed his studies at the University of Sydney in 2007 and undertook specialist physician training at Westmead Hospital before completing an Endoscopy Fellowship at Concord Repatriation General Hospital in 2015.

He joined The BMI Clinic in 2016 and took on a leadership role, greatly expanding the multidisciplinary team and developing a unique, comprehensive lifestyle programme to complement bariatric endoscopic interventions. One of the defining achievements has been the development of the world-first weight management programme, BMI Active, which allows real-time weight monitoring of all patients to enable proactive management and enhanced weight loss results.

Dr Sartoretto has achieved a number of Australian firsts, including:

HOW DOES THE ESG WORK?

The ESG procedure will shorten the length of the stomach by approximately 50% and will reduce its volume by 75%. Given this, the ESG works in several ways to help you lose weight:

  • The quantity of food that you can eat is significantly restricted – helping you to reduce your portion sizes
  • Your stomach will empty more slowly – helping you to feel fuller for longer
  • The number of calories your stomach can absorb is reduced


IS THE ESG PERMANENT?

The ESG is considered a durable procedure, rather than ‘permanent’. The stitches may break over time if they are over-burdened (due to over-eating or persistent vomiting), however scar formation within the stomach typically reinforces the stitches. Studies suggest the ESG is durable at the 5 year mark, and the ESG performs as well as the surgical sleeve in terms of weight loss over 18 months.


HOW IS THE ESG PERFORMED?

The ESG will be completed by your Bariatric Endoscopist within 50 minutes in a day hospital, under general anaesthetic. You will be discharged from the hospital on the same day.

The procedure is endoscopic, which means no incisions are required. Instead, a flexible tube with a camera (endoscope) is inserted into the your stomach via your mouth. Several rows of stitches are then made within the stomach using a special tool, making it very narrow.


HOW CAN I ENHANCE MY WEIGHT LOSS?

The ESG is a tool to aid weight loss, giving you a kick-start by allowing you to control portions as you develop healthy and sustainable habits. We know the ESG is most effective when accompanied by a lifestyle program, which is why at The BMI Clinic, the ESG is paired with either our Silver, Gold or Platinum lifestyle program, delivered by a team of weight loss specialists, including Bariatric Physicians, Nurses, Dietitians, Exercise Physiologists and Behavioural Coaches. Data from our own clinic demonstrates the more a patient engages with the team, the greater their weight loss.


HOW MUCH WEIGHT CAN I EXPECT TO LOSE WITH THE ESG?

The more you engage with your team, the more successful your weight loss journey will be. You can expect to achieve 20-25% TBWL (Total Body Weight Loss; ie. a 100kg person could expect to lose 20-25kg on average), however ultimately, the amount of weight you lose and maintain will depend on how closely you follow the advice of your medical and allied health care team.
